What can cause blood pressure to spike and lead to heart attack?

I am 41 and have suffered from spiking blood pressure from what seems to be no apparent reason for 10 years. I can have normal bp and then it spikes to over 200/100. This has been happening for over 10 years and getting more frequent. (With hospital visits and tests galore) I had a heart attack about 6 weeks ago (had a stent put in, 90% blockage) and it is possible that I have Dercums disease (Painful fatty lipomas everywhere although I am not obese) My diet is generally really good. I have increasing anxiety and all aspects of Metabolic Syndrome. (Type 2 diabetic, Extremely fatty liver) I have been fighting to get my health better for so long and have focused on my lifestyle to make things as good as I can. I would love to be around to see my children grow but it seems so hopeless after fighting for so long to still end up here. I am still having chest pain since my heart attack and have had 2 episodes of spiking bp since my heart attack. (going from e.g. 106/66 to 202/100) Thanks
