L5/S1 damage; excruciating constant pain down left leg to my foot

I've got L5/S1 damage.
Strong painkillers are no longer working. I have had a steroid injection under CT scan into my lumbar spine which has not reduced the pain at all.
What type of surgery is necessary to rid me of this debilitating pain? -
