Overnight acne covering the back

My 22-year-old son has been unwell for a few weeks. Over the past few days to week he has had bad overall body pain/aches and fatigue, sleeping up to 12 hours a night. This morning he woke up with spots all over his back that present like pimples, red and raised, some with pustules. He has never had this before, although he did have acne around 7 years ago. We thought it may have been chickenpox or shingles, however, the GP said it's acne.
Is that possible? -
