How can I help my partner with Type 1 Diabetes?

My partner has had diabetes for over 12 years now (he is now 25). Since being diagnosed, his diabetes was extremely poorly managed. He has improved a lot over the most recent years, however, has a long way to go. He lacks motivation and commitment to change his ways for his health. This is very frustrating to watch as his partner, and I don't know how far is too far to push him. I get to the point sometimes where I feel like saying I won't be with him if he doesn't take this seriously because I don't want to outlive my partner by 20+ years as a result of his attitude now. Should I keep trying to initiate change and make suggestions or let him figure it out on his own? He is not proactive about anything, and I am the exact opposite. So it drives me nuts seeing him do nearly nothing to help himself/taking things very slow. I don't feel this is something you can gradually make little changes. Yes, little changes are better than no changes but we have a lot to make up for.
