Not always. Small, symptom-free hernias can sometimes be safely monitored. However, hernias do not resolve on their own and generally enlarge over time. Surgical repair is recommended when a hernia causes pain, limits activity, or is at risk of complications such as incarceration or strangulation, where trapped bowel loses its blood supply.
