Knee is now giving away regularly, why?

I have torn my medial meniscus three time resulting in having most of it removed. Now after 10 years and no proper recovery, I have a narrowing in that side of my knee and also pretty bad muscle wasting away. I would exercise every week with physio and continue to exercises at home, but it seems to be getting worse faster than it is getting better. Last Thursday I had a big fall on it where my leg went from under me. It felt like it was going to dislocate, now I have a lot of swelling but also feel a hard bone like lump next to the patella on the medial side. I have a surgeon appointment tomorrow but does anyone have any idea what's going on?
