My teeth have moved but retainer doesn't fit. What can I do?

Hi! I have had my braces off for about 4 years now, my first retainer broke after 2years because I clench my teeth down hard when I sleep so I got a new one made. It's been about a year since I have worn the 2nd retainer - a) it was made too small, I don't know how but even when I first put it on my teeth it was way too tight and it cut into my gums at the bottom b) he has now moved and I can't seem to find his number anywhere to contact him and see if he still has my very first mould so I can move my bottom teeth back into place! My tops are perfect, my bottoms have moved a lot. Only one tooth that points out and it's right at the front as well. It makes me so self conscious when I talk because the problem was fixed until he didn't make my retainer correctly. What can I do?
