Thanks, I am in North east Vic which means I will need to go to Melbourne. Should I get a referral for somewhere like The Epworth or RMH or could somewhere like Avid Allergy help (ie. do more than just skin prick tests) Thanks

I am not that familiar with the Victorian system but all three places that you mentioned look good to me. As you wrote, a bit more than skin prick tests seems to be needed to figure out what is going on and then deal with it

The first thing that jumps out to me with this case is that you have clinical features that certainly sound as if they are consistent with chronic rhinitis. You report having trialled conservative strategies with the daily antihistamine, Rhinocort, and some degree of allergen avoidance strategies. You also report that an allergist has confirmed multiple sensitizations on skin prick testing. So I feel that the next step for you would be a review of everything you're currently doing, to ensure that it's done correctly, while also adding on allergen immunotherapy. I can see the other elements there include idiopathic anaphylaxis for which you carry an EpiPen, and there is benefit to having a second opinion on that as there are other allergens that are not typically tested initially, including alpha-gal, omega-5-gliadin and Pru-p3 to name a few. Finally, your joint pain with sinus symptoms does raise the remote possibility of an autoimmune condition, and overall an immunologist review would be recommended.
