What can I do for a terrible rash under my bust?

I am a big lady, and have a very big bust. I get a terrible rash under my breast, and it smells horrible, even if the rash is as small as a child's fingernail. I have tried everything possible to get rid of it and prevent it. Does anyone have any answers for me please? Thank you in advance. -
