Knee malalignment occurs when the leg’s mechanical axis deviates from normal, causing excessive load on one side of the knee. Varus malalignment (bow-legs) overloads the inner (medial) compartment; valgus malalignment (knock-knees) overloads the outer (lateral) compartment. This uneven loading can accelerate cartilage wear and lead to compartmental arthritis, particularly in younger, active individuals.
